Answered questions and filled out an ADIME note on Mrs. Joaquin, a 24-year-old Pima Indian who has had type 2 diabetes mellitus since age 13. Mrs. Joaquin has experienced a declining glomerular filtration rate for the past two years. She is being admitted in preparation for kidney replacement therapy.
Answered questions and filled out an ADIME note on Mary Corbet, a 60-year-old divorced Caucasian female with complaints of memory loss, insomnia and general fatigue. She was recently asked to “retire” after having worked there for 30 years. Lately, Mary has not been herself, with trouble sleeping, increased back pain, increased alcohol use and difficulty concentrating.
Answered questions and filled out an ADIME note on Meredith Dewietz. Meridith comes to the clinic today with complaint of long standing symptoms including numbness, tingling, weakness, lack of coordination, clumsiness, and impaired memory. She has had these symptoms for months, beginning before her recent hospitalization for a bleeding ulcer.
