Nutritional Management of Diabetic Gastroparesis: Counseling and Education Interventions

The Counseling Case Report project will provide the intern practice of the Nutrition Care Process (NCP) through the lifecycle and analysis and application of Motivational Interviewing counseling techniques in a client case. The case report will show evidence of the intern’s application of the NCP and counseling techniques in the nutrition intervention.

Diabetes mellitus (DM) is group of diseases characterized by high blood glucose concentrations resulting from defects in insulin action. Type 2 diabetes mellitus (T2DM) is a form of DM where the body either does not produce enough insulin or is insulin resistant. About 13% of adults in the United States have type 2 diabetes. Type 2 diabetes has the potential to cause many complications including diabetic gastroparesis, a syndrome of delayed gastric emptying. Diabetic gastroparesis occurs in 1% of patients with type 2 diabetes. One such case is that of a patient with a 7-year history of type 2 diabetes. She had previously been diagnosed with diabetic neuropathy and was most recently diagnosed with diabetic gastroparesis. Her comorbidities include stroke, obesity and hypertension. After her diagnosis, the patient returned to see the Registered Dietitian and Certified Diabetes Educator to determine the most appropriate nutrition therapy to help manage her diabetic gastroparesis while monitoring her blood glucose levels and continuing to lose weight. Through a client-centered plan and individualized implementations of specific nutritional and counseling interventions relating to diabetic gastroparesis, type 2 diabetes and weight loss, a care plan was developed to help the patient reach her nutritional goals while improving her overall health status.

Importance of Drinking Adequate Milk During Pregnancy

The Community Needs Assessment and Intervention Project will provide the intern an opportunity to gain experience applying the NCP in a community setting. The intern will perform a needs assessment, utilize the assessment data to identify a problem in the targeted population, implement a related intervention, and evaluate the effectiveness of the intervention.

The OB/GYN Clinic provides a variety of health care services to women, including prenatal nutrition counseling. The community of Harlem has a primarily Black and Hispanic population. It is crucial to understand the cultural beliefs and attitudes of these populations in order to best service them.

Based on conversation with both patients and the dietitian at the OB/GYN Clinic, it was determined that patients at the clinic were not drinking enough milk or consuming adequate protein and calcium during pregnancy. This could be due to cultural preferences, preferences due to pregnancy, or other causes. It was deemed important to address this dietary gap due to the importance of drinking milk to have adequate protein and calcium intake during pregnancy.

An education session on the importance of drinking adequate milk during pregnancy was given to pregnant patients at the OB/GYN Clinic. These education sessions were given to patients individually while they waited to see their OB doctor. Results of this session showed that after receiving the education, 90% of participants were able to name at least two benefits to drinking milk, and 71% of participants who were not currently drinking the recommended 3 cups of milk daily stated that they might increase their milk intake.
